We spend a lot of time focusing on our jawlines and chins, but nothing reveals the structural aging of the lower face quite like the neck. Specifically, many patients come to our office frustrated by vertical “cords” or bands that run down the front of their neck.
These bands can make the neck look strained or aged, even if the rest of your face looks youthful. The good news is that these aren’t loose skin, and they aren’t fat—which means we can treat them very effectively with a procedure called a Platysmaplasty.
Anatomy 101: The “Hammock” of the Neck
To understand why these bands form, visualize the neck’s anatomy. The platysma is a thin sheet of muscle that spans the front of your neck.
I often describe the platysma as a hammock. Its job is to sling under the jawline and hold up the deeper structures of the neck, keeping everything tight and high.
In our youth, this hammock is taut and smooth. But as we age (or sometimes due to genetics in younger patients), two things happen:
- The muscle weakens and separates in the middle.
- The edges of the muscle flare out.
Those vertical bands you see in the mirror? You are actually looking at the edges of that separated muscle “hammock” hanging down.
What is a Platysmaplasty?
Because the problem is muscular, creams and skin treatments won’t fix it. The solution is a Platysmaplasty.
During this procedure, we surgically tighten this muscle hammock. We bring the separated edges of the platysma back together and tighten them, effectively “slinging” the contents of the neck back up under the jawline. This restores the structural support of the neck and creates that sharp, chiseled angle that defines a youthful profile.
Who is the Ideal Candidate?
This procedure is versatile and fits into treatment plans for different ages:
- For Younger Patients: If you have good skin elasticity but have always had visible bands or a “soft” neck angle, a platysmaplasty alone can be an excellent solution. It tightens the neck without the need for extensive skin removal and is a simple procedure often performed under local anesthesia.
- For Patients with “Heavier” Necks: If you have significant fullness, aging, or heavy tissue in the neck, tightening the hammock alone might not be enough. In these cases, a platysmaplasty is performed as a crucial part of a Deep Neck Lift. We reduce the deep fat and heavy structures first, then tighten the platysma over them to contour the shape.
Why Liposuction Alone Isn’t the Answer
A common misconception is that neck fullness or banding can be fixed with simple liposuction.
While liposuction is great for removing superficial fat, it does not tighten muscle. If you have platysmal bands and you only perform liposuction, you might actually reveal the bands more by removing the cushion of fat that was hiding them. To get a smooth, flat neck, we must address the muscle directly.
