Patients frequently come into the office asking for a specific treatment they saw online, only to find out that a completely different procedure is actually the best fit for their anatomy. Whether you are looking for a subtle tweak before a wedding or a permanent structural change, understanding the difference between a Lip Flip, Lip Filler, and a Lip Lift is the first step.

1. Lip Flip

A lip flip is a quick, non-surgical treatment where a very small amount of neurotoxin (like Botox or Dysport) is injected into the muscle just above your upper lip. This relaxes the muscle, allowing the top lip to gently roll outward and upward, exposing more of the pink lip.

  • Impact: It is the most subtle option available. It does not add any actual volume to the lips; it simply creates the illusion of a slightly fuller upper lip.
  • Who it’s for: This is most popular in our practice among patients under 50. It is fantastic for patients who already have good natural upper lip volume and are just looking for a little more “pop” for an upcoming event.
  • The Catch: Because the mouth is a highly mobile area, the neurotoxin wears off quickly. You can expect a lip flip to last about 2 to 3 months.

2. Lip Filler

Lip filler involves injecting a hyaluronic acid (HA) gel directly into the lips. Unlike a lip flip, filler physically adds substance to the tissue.

  • Impact: Filler is incredibly versatile. It strictly adds volume and cannot shorten the white part of your upper lip (the skin between your nose and your mouth), but it can brilliantly revolumize, balance, and improve the symmetry of both the top and bottom lips.
  • Who it’s for: Filler looks excellent in both younger and older patients. Whether you want a plump, hydrated look or need to restore volume lost to natural aging, filler is the gold standard.
  • The Catch: It requires maintenance. While it provides a moderate, predictable impact, most patients find their filler looks best for about 6 to 12 months before needing a touch-up.

3. Lip Lift

As we age, gravity and a loss of skin elasticity cause all of our upper lips to lengthen. The pink part of the lip rolls inward, and the white skin above the lip gets longer. Neither a lip flip nor lip filler can fix this structural elongation. That is where a lip lift comes in.

  • Impact: A lip lift is a minor surgical procedure where a small strip of skin is removed from just beneath the nose. This lifts the upper lip, permanently shortening the white lip and rolling the pink lip outward for a youthful, voluminous look.
  • Who it’s for: This is the most powerful and longest-lasting option (it is permanent, though you will continue to age normally). It is ideal for patients bothered by a long upper lip or those who feel filler makes them look “ducky” because their lip has nowhere to go but out.
  • The Catch: Because it is a surgery, there is a scar hidden in the natural crease under the nose. Because of this scar, we generally only offer this procedure to patients who have enough white lip length to make a significant, worthwhile improvement.
  • Rule of Thumb: Every face is unique and we tailor our recommendations carefully, but generally speaking, you need about 17–18mm of length in the central white lip to be considered a good candidate for a lip lift.

The Bottom Line

If you want a cheap, quick tweak for a weekend event, try a Lip Flip. If you want customizable volume and symmetry that lasts up to a year, opt for Lip Filler. If your upper lip is long and you want a permanent, structural change, a Lip Lift is likely your best investment.
