Can I Get a Neck Lift Without a Facelift?

One of the most frequent questions I hear during consultations is, “I like my face, I just hate my neck. Can we just fix the neck?”

The short answer is: Yes, in some cases.

However, the longer answer depends entirely on your anatomy and where your specific signs of aging are occurring. While an isolated neck lift is a fantastic procedure, it is not the right tool for every patient. Here is how I determine if “neck only” is right for you.

The Ideal Candidate: Younger Patients (less than ~40–45)

For patients generally under the age of 40 to 45, a solo neck lift can be a great option.

In this age group, patients often come in with “heaviness” or genetic fullness in the neck, but their jawline and cheeks are still relatively youthful. They haven’t yet developed significant jowling or sagging in the lower face.

If your aging changes are strictly isolated to the neck—meaning you have good definition along the jaw but excess fat or muscle banding below it—we can perform a neck lift to contour that area. This restores a sharp angle without touching the face, providing a harmonious and natural result.

The Risk: The “Swept” Look

Once we move past that ~45 age range, the jowl fat pads begin to drop in most individuals, and the strategy must change.

The danger of performing a neck lift by itself on a patient with visible jowls is that it can create an unnatural or distinct disconnect. Imagine tightening the skin and muscle of the neck, while leaving the heavy, sagging tissue of the jawline and cheeks (the jowls) untouched right above it.

This often results in a “swept” look, where the neck looks incredibly tight and youthful, but the face looks heavy and draped over it. Instead of looking rejuvenated, the patient looks “done,” because the neck no longer matches the face. For this reason, performing a solo neck lift on a patient with jowling is not my preference.

The Solution: Blending the Result

For patients who are worried about their neck but also have early signs of jowling, the best approach is often a Lower Facelift combined with a Neck Lift.

This doesn’t mean you need a “full” facelift or that you will look pulled. It simply means we are addressing the jawline and the neck as a single aesthetic unit. By lifting the jowls back into their youthful position while tightening the neck, we blend the result seamlessly.

This approach ensures that your sharp new neckline transitions naturally into a defined jawline, keeping you looking like a refreshed version of yourself—not like someone who had “work done” on just one body part.
