One of the most common complaints patients bring to the consultation room is the “turkey neck.” While it’s a term nobody likes to use, it vividly describes a very specific type of aging: the loss of definition and the relaxation of the central neck and jawline.
Many patients assume that this loose neck tissue is simply a matter of excess skin or a little extra fat. Consequently, they often spend years—and significant money—trying to fix it with superficial treatments like liposuction or non-invasive skin tightening devices.
Unfortunately, for a true “turkey neck,” these treatments rarely work. Here is why that happens, and what actually works to restore a sharp, youthful jawline.
Why Lipo and Skin Tightening Aren’t Enough
To understand the cure, you have to understand the cause. In most cases, a heavy or sagging neck is not just skin deep. It is caused by the effect of gravity on the deeper structures of the neck.
Specifically, we are talking about the muscles and fat located under the chin and deep beneath the platysma muscle. Standard liposuction only targets the superficial fat layer (just under the skin). If your fullness is caused by deep fat or muscle sagging, liposuction will not reach the problem area. In fact, removing superficial fat without addressing the deep structures can sometimes make the neck look older or “hollowed” without fixing the bulge.
The Solution: The Deep Neck Lift
To treat the actual problem, we perform a Deep Neck Lift.
Unlike a traditional lift that might only pull back the skin, a deep neck lift reshapes the foundation of the neck. During this procedure, we address the heavy anatomical structures that have succumbed to gravity:
- Digastric Muscles: These are the muscles located under the chin. Over time, they can bulk up or sag, contributing to fullness. We tighten or reduce them to flatten the submental area.
- Deep Fat: We remove the fat pads located deep within the neck that liposuction cannot reach.
- Submandibular Glands: In some patients, these glands contribute to a bulge on the sides of the neck. They can be reduced or lifted to help restore a sharp, clean contour.
By sculpting these deep layers, we aren’t just stretching skin; we are anatomically restoring the sharpness under the chin.

Recovery and Patient Experience
This might sound like a dramatic procedure, but patients are often surprised to hear that the recovery for a deep neck lift is quite manageable.
- Drains: We do not use drains for this procedure, making the post-operative experience much more comfortable.
- Timeline: You can expect roughly 2 weeks of swelling and bruising.
- Comfort: Tightness is expected, but most patients report minimal pain, which is easily managed with over the counter medications in most cases.
Is It Safe?
A common question patients ask is whether it is safe to reduce these deep structures. The short answer is – yes. Like any procedure, a deep neck lift carries risk of bleeding, infection or damage to surrounding structures. However, when a deep neck lift is performed conservatively by a facial plastic surgeon with experience with this procedure, these complications are extremely rare. Reduction of the digastric muscles or submandibular glands for aesthetic contouring has not shown to cause functional impairment to speech or swallowing down the line.
Overall, this is a safe, highly effective procedure that offers a long-lasting result because it treats the aging neck at its source.
